Lighthouse Family Chiropractic believes in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases.

Insurance

Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare. Since there are so many plans and coverage varies widely, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits before treatment, at no charge.

Payment Options

Lighthouse Family Chiropractic accepts Cash, Check, Visa, Mastercard, Discover, and American Express.

Our Financial Policy

We are committed to providing the finest chiropractic care and is pleased to discuss our professional fees with you at any time.  Your clear understanding of our financial policy is important to our professional relationship.  Please ask any questions about our fees, financial policy, or your responsibility.

All patients must complete and sign our patient information forms in full before being seen by Dr. Kathy Lewis-Padgett.

Co-payments and/or deductibles are due at the time of services if we participate with your insurance carrier.  If we do not participate, then the total fee is due at the time of service.

We will help you receive the maximum allowable benefits from your insurance company.  If we do not participate with your carrier then 100% of the fee is due at the time of service!  We will assist you in filing claims, but we will not become involved in disputes between you and your insurance carrier regarding deductibles, co-payment, covered/non-covered charges, secondary insurance, "usually and customary" charges, or third party liability.  All charges are coded with the standard government "CPT" codes.

A fee of $25 will be added to all returned checks.
